Home & Health Care Management Mission Statement

​It is our mission to provide innovative, quality, professional health care, counseling, educational and wellness services to the communities we serve in a caring, cost-effective manner.

The Centers for Medicare & Medicaid Services is in the process of issuing new Medicare cards to help protect the identities of beneficiaries by removing Social Security Numbers from Medicare cards. CMS will be mailing new Medicare cards to beneficiaries in April 2018. The cards will be mailed in seven waves by groups of states over the next year. All beneficiaries should receive a new card by April 2019.
